Do i have to list my last employer on application?

If I only worked for them for 3months, as an on-call nurse? I only worked 10days out of the month, and it was a really bad experience so I don't want to. But will they know?

1. you don't have to. just write references available upon request
2. Legally speaking, in USA, you can list the HR department of your last job. They are obliged to confirm the dates you were working as on-call nurse. Not your job performance, whether you got along with co-workers etc.
3. It is an offense to withold anything asked on any application -this could be prosecuted
