Non-Web based Mailing list form?

I'm trying to find a decent mailing list form that will export the information to EXCEL. Here's the scenario: I'm going to have a laptop at a trade show that i want people to fill out. I'd like for them to answer about 10 questions, hit SUBMIT and then have the form reset itself for the next user. I then want to take all the gathered information and put it into an excel sheet or CSV. The only thing is that since I'm not sure if I will get a web signal that day, it can't be hosted on a website. What kind of program can i use for this? Thanks! RO

  1. You can do this really pretty simply with MS Access. You will need to find someone that has the knowledge to create a database and format the fields to accept the information you want and you can easily export the info to an excel spreadsheet. This is not a case of high powered programing but you will want to find someone that creates the DB not just or is only a user of Access. You can have the DB architect create a form (it's part of functions in the Access objects list) for entering info and a submit button to submit on the form. This will save the info clear the form and be ready for the next user. It's really no big thing. This could probably be done in about 90 minutes.
