PLEASE HELP..I want to know why my added drop down lists arent working on the PDF file I just made. Thank You?

I've been working on creating a PDF file (work order) for work and now that im finally finished my drop down lists aren't working. If you could please give me some advice or let me know what i'm doing wrong then I would greatly appreciate it. I need an answer ASAP and BTW I'm Using Adobe 9.0 Professional?
