  1. Well, it really all depends on what kind of book or material you're using. I would say C. The reason that I pick C over D is that you usually should put in the numbers of the pages that you used, but not the entire book page count. Here's a few sites that I've used in the past.

  3. C is likely the best answer; however in part it depends on the style manual being used. If it's just for a school paper, most likely it will be MLA. However, there are also APA and Chicago, to name 2 more. Each has its own rules and format.
