Is there anywhere I can find a template for Ltd Company Board Meeting minutes?

I imagine its a pretty straight forward structure but it would be much appreciated if I could get a list of the key headings, or a link to an online template.

  1. You want an agenda, which will just have your company name, proposed date of meeting and the points you wish to discuss, with 'any other business' at the end. Minutes should show Date of meeting, who was present, what was discussed and who is responsible for actioning any targets set, normally shown in a right hand column state initials of that person. Also include when points should be actioned by, this would then be measured at the next meeting.
