Could anyone provide me a legal complaint template, complete with numbered margins?

I am attempting to file a "pro se" civil complaint and I can't seem to find a template for the standard format used by most courts, with the parties listed at the top in brackets, and the numbered margins. Where can I find this?

1. The exact format of legal pleadings varies from state to state and you do not say where you are. The format is usually found in the state "rules of court." If you use Microsoft word as your word processor, you can go to the Microsoft website and download a template for legal pleadings.
