How do I write an explanation of negative items on my credit report for a mortgage?

I'm not sure what or how to write this letter. The mortgage company needs one, but I'm not sure if I am supposed to write it in list form or in letter form. There's nothing major and it all stems from when my husband was in the Army and there were serious pay issues (like not getting paid at all! lol). How do I go about writing this letter?
